Rhinoplasty

Rhinoplasty is surgery to improve the shape or appearance of your nose and can help make breathing easier.

People who are either unhappy or dissatisfied with the appearance of their nose which can arise from birth imperfections to injuries, or even aging, are candidates for rhinoplasty, or nose reshaping surgery. A traumatic injury to your nose can change its shape and make it more difficult for you to breathe and many people often undergo rhinoplasty to correct this.

Everyone’s nose is different and similarly everyone’s perception of the ideal nose differs. A consultation with Dr Broadhurst will include a discussion with you about the desired appearance of your nose, an examination of your nose and photographic analysis. An individualised operative plan unique to your needs will be formulated and discussed with you at your consultation. This will give you an idea of what can be achieved through nose surgery.

Rhinoplasty is an operation that alters the underlying framework of your nose thereby enhancing its appearance and making breathing easier.

Your nose is built with a scaffolding of bone and cartilage which is what gives your nose its shape. The framework is then covered with skin and the thickness of skin varies in different parts of the nose and from one individual to another. The skin covering your bone and cartilage framework gives you the nose you see in the mirror.

A rhinoplasty can increase, decrease or reshape any of the cartilage or bony components of the framework of your nose but little can be done to change the skin.

Anyone who is unhappy with the appearance of their nose or who has difficulty breathing through their nose.

There are many reasons why you may be unhappy with the appearance of your nose:

Congenital or birth imperfection – this “defect” will generally have been present since childhood and sometimes becomes more noticeable in adulthood. Some people are also just born with unusually shaped noses. Most children have “cute” noses but in adolescence our bodies change and this includes the nose. Sometimes your nose can develop a bump or hump, the tip may stick out too far, your nose may become too long, or too big. A rhinoplasty can improve these unwanted features.

Trauma – trauma can be recent or in the past. An injury can alter the framework of your nose and make it look bent or twisted. Often this is associated with breathing difficulties.

Airway – some people have difficulty breathing through their nose. There are many causes for this. In some circumstances your airway can be obstructed by bone or cartilage that is the wrong shape or in the wrong place. A rhinoplasty can correct this and make breathing easier.

Aging – there are gradual changes in our noses with age and these are most obvious at the tip. The tip of the nose can look bigger, longer or droopier with increasing age. In some cases the skin can become thicker. A rhinoplasty will improve the shape of your nose, correcting the length and droopiness, but little can be done to alter the skin.

During your consultation with Dr Broadhurst he will review your past medical history including any previous nasal operations and any medications you may be taking.

He will then examine your nose and facial structure. This analysis will determine the cause of any breathing difficult and will also allow a determination of the factors contributing to your current nasal shape and how these can be altered to improve the appearance of your nose.

Only after a complete physical examination can Dr Broadhurst determine if the changes you want are possible. While significant changes are possible you can’t have a “new nose,” only a “new shape.”

A second consultation will then be scheduled prior to your surgery to ensure you feel comfortable with your decision, to ensure your complete understanding of the procedure and to give you the opportunity to ask any further questions.

A rhinoplasty can generally be performed as a day surgery procedure.

Incisions will be made on the inside of your nose and the cartilage and bone framework restructured to improve breathing and give your nose its new shape. Upon completion the incisions will be closed with dissolving stitches and a plaster splint applied. You will need to keep this clean and dry until it is removed in seven days.

The operation itself will take between one and three hours depending on the level of complexity and once completed your anaesthetist will wake you.

After surgery you will be taken to the recovery room where you will be closely observed by our experienced nursing staff. They will take care of you until you are fully awake and ensure that you are comfortable.

You will be allowed to go home once you have fully recovered from your general anaesthetic which is usually three hours after the operation is completed.

After your operation it is very important that you remain in the care of a responsible adult who will stay with you overnight.

Your anaesthetist will prescribe pain killers for you to take in the days following your surgery. There will be mild to moderate discomfort following your operation and it is advisable that you take it easy for the first week. An appointment will be made for you to see Dr Broadhurst one week after your operation to remove your splint.

The degree of swelling after a rhinoplasty varies from one individual to another and depends on the extent of your operation. Swelling and bruising can vary from barely noticeable to significant swelling and black eyes. You will be given detailed post-operative instructions at the time of your discharge.

It is important to avoid bending over or heavy exercise in the first week following your operation as these can increase the amount of swelling. Typically the swelling and bruising will resolve in seven to 10 days.

There are risks involved with any operation and these include the anaesthetic and the operation itself. If you are fit and healthy then the risk of complications is generally very low and Dr Broadhurst will discuss all risks with you prior to your surgery.

As with any surgical procedure there are limitations. The aim of the consultation process is to determine those features of your nose which you would like altered, the expected outcome of the surgery and for you to determine if this is what you want.
